Dear Flight Attendant,

Pursuant to Section 21.of the Flight Attendant Agreement, you are being recalled from your voluntary furlough effective March 15, 2004. This recall is necessary due to an increase in aircraft hours in response to schedule adjustments and an increase in charters. Although we recognize that you will now be returning from your voluntary furlough earlier than expected, this recall is based upon the creation of more jobs and more flying, which is good news for our division and our company.

Following your official acceptance of the recall via your returned Recall Notification (enclosed) by Fed Ex or Certified Mail, there will be three additional actions for you to complete in preparation for your return to work. The first relates to the criminal history record check and badging, the second to your medical clearance, and the third to the scheduling of training. Please carefully review the Voluntary Furlough for Flight Attendants information package and this letter prior to your return. You may view the furlough package on SkyNet via the Onboard Service web site at united.intranet.ual.com.

Criminal History Record Check / Badging

In accordance with Code of Federal Regulations, you must complete, sign and date the enclosed Criminal History Record Check Continued Disclosure Statement and fax the form to WHQUN at (847) 364-2445 no later than Feb. 2, 2004.

  • This is an acknowledgment of your continued obligation to notify United Airlines within 24 hours if you are convicted of any of the 36 felonies identified by the Transportation Security Administration (TSA) that would make you ineligible for Security Identification Display Area (SIDA) access.
  • Your United Airlines (TSA) crewmember ID badge (required for access to domiciles and all secured areas) cannot be provided to you until we are in receipt of your signed Continued Disclosure Statement.

Additionally, if you have been away from work longer than 365 days AND more than 365 days have passed since the date of your most recent Criminal History Record Check clearance, your fingerprints will be resubmitted. These must be cleared prior to issuing your United Airlines (TSA) crewmember ID badge. Resubmitting your fingerprints does not require any action on your part; this process will be completed by WHQSW. We anticipate this will not impact your scheduled return to work date. In the event that your fingerprints do not clear or further information is needed, you will be contacted by a United representative with information on how to proceed.

Medical Clearance

Complete, sign and date the enclosed Statement of Health form and fax it to WHQUN at 847-364-2445 no later than Feb. 2, 2004. You will be able to fly only if the Statement of Health form is received and you are deemed medically able to fly.

  • If you are unable to return on March 15, 2004, due to medical reasons, you must substantiate your medical condition by personally meeting with your Company Medical Doctor at the United Medical Office for your domicile. The deadline for meeting with your Company Medical Doctor is Feb. 16, 2004. Failure to substantiate your medical condition by the deadline will be considered a failure to return to service per Section 21.F. of the UAL/AFA Agreement and you will be separated from United Airlines effective March 14, 2004.
  • If you are not medically able (e.g. hospitalized, final stage of pregnancy, etc.) to personally meet with your Company Medical Doctor at the United Medical Office for your domicile, your treating physician must contact your Company Medical Doctor by Feb. 16, 2004. Failure to do so by the deadline will also result in the application of Section 21.F. of the UAL/AFA Agreement and you will be separated from United Airlines effective March 14, 2004.

Training / Qualifications

To determine your individual training requirements, schedule training and determine travel arrangements, contact the Flight Attendant Service Center through FLT-LINE option 4/2. The training you need will include one or more of the following: Refresher Flight Attendant Training (RFT04), Service Requalification Training (SVCRQL), Recurrent Emergency Training (RET), Requalification Emergency Training (RQL), Cabin Defense Security Training (CDS1), and Automated External Defibrillator Training (AED).

It will be necessary for you to attend either Refresher Flight Attendant Training (RFT04) or Service Requalification Training (SVCRQL) prior to attending any other training programs (RET, RQL, AED, CDS1), based upon the length of time that has lapsed since your last trip. You will need to schedule yourself by Feb. 11, 2004 to attend Refresher Flight Attendant Training (RFT04) or Service Requalification Training (SVCRQL). If you have not scheduled yourself by Feb. 11, 2004, you will be assigned a training class.

Refresher Flight Attendant Training - RFT04 (if absent from flying for 12 months plus 1 day-36 months)
As of March 1, 2004, if you have not flown within the past 12 months plus one day up to 36 months, Refresher Flight Attendant Training is required in order for you to return to work. This is a one-day qualification training program conducted at WHQIT (Training Center). You must attend this training prior to attending any other training programs (RET, RQL, AED, CDS1). During Refresher Flight Attendant Training, you will receive pertinent information regarding Onboard Service policies, procedures and initiatives such as Ted, Buy On Board, etc., as well as your United Airlines (TSA) crewmember ID badge, Flight Attendant Operations Manual (FAOM) and bidding information.

Service Requalification Training - SVCRQL (if absent from flying for 36-60 months)
If you have not flown from 36 months up to 60 months, Service Requalification Training is required in order for you to return to work. This is a three-day qualification training program conducted at WHQIT (Training Center). During Service Requalification Training, you will receive pertinent information regarding Onboard Service policies, procedures and initiatives such as Ted, Buy-on-Board, etc., as well as your United Airlines (TSA) crewmember ID badge, Flight Attendant Operations Manual (FAOM) and bidding information.

Recurrent Emergency Training (RET)
If your training month is January, you will need to schedule yourself by Feb. 11, 2004, to attend RET during the month of February. If you have not scheduled yourself for RET in February, you will be assigned a February training class to avoid the need for an additional day of emergency training in the month of March or beyond.

Requalification Emergency Training (RQL)
Requalification Emergency Training consists of one day of requalification training and two days of RET training. The requirement to attend this three-day emergency procedures training is based upon your RET month.

Cabin Defense Security (CDS1)
Cabin Defense Security I training is a self-study-training program, completed in two parts. This will take approximately three hours. You will view a videotape/booklet and complete a computer-based post-test. You may complete this training while you are at WHQIT (Training Center) or in a domicile LRC once your United Airlines (TSA) Crewmember badge has been returned to you.

Automated External Defibrillator (AED)
Effective April 12, 2004, the completion of AED training will be required in order to fly. This one day class is offered at the Emergency Procedures Training Centers (TK).

Authorization for Release of Anti-Drug and Alcohol Misuse Prevention Program Information
As required by Department of Transportation Regulation 49 CFR Part 40, Section 40.25 you will be requested to provide information, relevant to the last two years, regarding background checks by any previous employer(s) for drug and alcohol testing records. If you have worked in a safety-sensitive position during your furlough, please be prepared to provide employer and company address information while attending Refresher Flight Attendant Training (RFT04) or Service Requalification Training (SVCRQL).

Passport/Visa/Flying Qualification Update
Current passport and visa documentation is critical for your return to active work status. When returning to work, you should be prepared to present your passport to ensure your company records reflect your full and legal name.

For crew visa information go to the Onboard Service Division homepage on SkyNet. Select the Standards and Policies link. There you will find visa and passport information by country. If you have additional questions, contact your local HR representative or domicile coordinator.

Note: Please bring your valid passport with you when attending Refresher Flight Attendant Training or Service Requalification Training at the Training Center (WHQIT).

Schedule Bidding

Since you will not be available on the first day of the flying schedule month, you are ineligible to participate in the March bid. Partial month bidding information will be available to you during your training at WHQIT. These bids will close at 0800 Central Time on March 8. If you will be attending training March 8 or later, please contact Advance Schedule Planning through FLT-LINE option 5, to obtain a bid package by March 5.

Updated Uniform Orders

Your eligibility to place orders with Brookhurst will be updated on Jan. 28, 2004. Please ensure that you have your uniform garments prior to your return to work. To place an order, you may do the following after that date:

Procedure for BP3 Travel Authorization

This procedure applies to those attending Refresher Training (RFT04) at WHQIT (Training Center).

Follow these instructions to arrange transportation from the nearest United/United Express location, based upon your permanent address or domicile, to Chicago, O’Hare. Due to United Airlines' policy, if you are based at ORDSW, you will not be provided transportation, as training will be conducted in the city where you are domiciled.

  1. Call 800-UAL-LIST or access SkyNet at united.intranet.ual.com and choose the WebList link (use the “Positive Space Booking” tab at the top of the SkyNet WebList page) to make your positive space reservation.
  2. Note your record locator and flight numbers after completing the booking process.
  3. Call the Flight Attendant Service Center through FLT-LINE option 4/4 to authorize your travel. Provide the center with the following information:
    • Name and file number
    • Originating flight number
    • City you are departing from
    • Record Locator
  4. For United flight segment ticketing, proceed to the United ticket counter prior to your flight with the information from step #3.
  5. United bus service from O’Hare to the Training Center (WHQIT) departs from the lower level (baggage claim area) at door G every hour on the half-hour, Monday through Friday from 0630 to 2130.

If you require transportation from a United Express location, please allow the Flight Attendant Service Center seven business days to process your request and mail your tickets.

It is your responsibility to cancel any unnecessary flight segments and advise the Flight Attendant Service Center of changes made to your reservation.
