Doctor’s Note for Point Reduction

As part of the Attendance Management provisions of our Contract in Section 23.F.2, we have the opportunity for Flight Attendants to receive a .5-point reduction to the Attendance points given for absences due to illness or injury that are six (6) calendar days or less. To qualify for this .5-point reduction, Flight Attendants are required to obtain from their physician (or United’s medical department) a written note that includes the following required information and submit it to their supervisor:

  • Date of illness
  • Date of Examination (when you were seen)
  • Date of return to work
  • The signature of the physician or the physician’s designee is on a document that contains, at a minimum, the physician’s printed name, work address, and work telephone number.

IMPORTANT NOTE: A Virtual Doctor visit and note are not permitted for this point reduction. You must be seen in person by a physician.

Flight Attendants are never required to disclose their personal illness or injury to anyone other than United Medical, so ONLY the information above is required on the physician’s note (also known as a Doctor’s note).  If your note contains any confidential medical information such as a diagnosis, prognosis, the condition, after-care, etc, be sure to black out or cover that information before presenting the physician’s note to your supervisor or through HelpHub. 

Doctor’s notes must be submitted on or before your next scheduled assignment and may be presented in person or by emailing a scanned copy or photo of the physician’s note to your supervisor. You also have the option to submit your note through HelpHub>Flight Attendant Point Reduction Request. 

Please note it is important that the Doctor’s Note be explicitly given to your supervisor or through the HelpHub direction above, and not to United Medical.
