1095-C Form Availability

Purpose: 1095-C forms verify that you were offered health insurance by a large employer that meets Affordable Care Act (ACA) standards, thereby helping to determine if you owe a penalty or are eligible for tax credits.

These forms are typically sent by the end of February or early March for the prior tax year to employees working full-time for at least one month of the calendar year.

This form includes info on the employee, employer, months of coverage eligibility, and the cost of coverage and should be kept for your tax records. However, this form is not required to be filed with your tax return. However, you may need some information on this form to complete your taxes.

Starting with the 2025 tax year, United has changed the process to access your 1095-C forms. These forms will be available online through the company’s new Trusaic platform and you’ll receive an email once the forms are available, including instructions on how to access Trusaic.

Additional reminders

• All 1095-C forms will be available by the March 2, 2026, federal deadline.
